Passports

The Consular Section in Zagreb can assist you in replacing worn-out, damaged, expired, lost, or stolen passports; amend your passport; and issue a newborn's first passport.

Passport renewal overseas. All U.S. consular facilities worldwide can issue to U.S. citizens the latest, state-of-the-art passport with enhanced security features including a photo-digitized image.

The new passports can be manufactured only in the U.S. Therefore, passport applications are forwarded to the National Passport Center which then sends the prepared passports to consular facilities. The processing time is usually three to five working days.

U.S. citizens can apply for the U.S. Passport card, which can be used to enter the United States at land and sea ports of entry when arriving from Canada, Mexico, the Caribbean and Bermuda. The card may not be used to travel by air. Further information about applying for a passport card can be found on http://www.travel.state.gov/.

An applicant who was at least 16 years old when his or her passport was issued and holds either a passport issued within the last 15 years or a valid passport and uses the same name or has documents reflecting name change, may submit a renewal application by mail or in person. Those who don’t qualify to renew their passports by mail must appear in person.
The following is required:

  • A completed DS-82 passport application form
  • $75.00 for the fee. If you are mailing the documents ,please send them by a "value letter" ("vrijednosno pismo") which protects the money.
    If you come in person, you can also pay in kunas and by major credit cards.
  • Two photos 2" x 2" (5cm x 5cm) with face in front position on white background.
  • A current passport.

U.S. Embassies and Consulates will continue to issue passports in emergency cases. Such passports are limited in validity and cannot be extended. Bearers are required to exchange them for a full validity passport upon completion of their emergency travel, either via passport facilities in the U.S. or U.S. embassies and consulates abroad.

All Minors Under 16 Must Appear in Person to Apply for Passports  

To enhance the accurate identification of passport applicants and aid in the prevention of international child abduction and trafficking, effective Feb. 1, 2008, the Department of State's Bureau of Consular Affairs now requires the personal appearance of all minors under the age or 16 applying for U.S. passports.

Minors under the age of 16 must appear personally with their parents to establish identity, proof of citizenship and proof of relationship. Previous U.S. passports are not acceptable as proof of minor’s relationship to the parents(s)/guardians(s), but should be submitted with the application. The new passport application fee for a minor under the age of 16 is $60, plus the execution fee of $25 for a total of $85.

Parents/guardians need to submit one of the following:

  • Certified U.S. birth certificate with parents’ names; or
  • Certified Foreign Birth Certificate with parents’ names (translation, if necessary); or
  • Report of Birth Abroad (Form FS-240); or
  • Certification of Birth Abroad (Form DS-1350); or
  • Adoption Decree with adopting parents’ names; or
  • Court Order Establishing Custody; or
  • Court Order Establishing Guardianship.

If the name of a parent or guardian is other than that on these documents, evidence of legal name change is required.

Both parents must appear personally with the minor child.  If one parent cannot appear and the applying parent does not qualify to apply alone, then the parent who submits the application must also submit a signed, notarized statement of consent from the other parent. Parents may use the Statement of Consent: Issuance of a Passport to a Minor Under Age 16 form (part 5) - (DS-3053).

Replacement Passports:
To obtain a replacement passport (damaged, lost or stolen passport) you will need:

Stolen Passports: If your passport has been stolen, you should first report the theft to the local police and obtain a written declaration from the police. Also, for a lost or stolen passport, U.S. citizens must complete and submit the DS-64: Statement Regarding Lost or Stolen Passport, available from the Consular section or click here.
 

Passport Amendments:

Passport Services can no longer amend valid passports. Instead, customers requesting name changes, extension of validity for limited passports, or correction of a printing error will apply for replacement passports.

If the change is within one year after issuance, they must submit a U.S. Passport Re-Application Form (DS-5504), together with the passport to be changed, the documentation required and new passport photos. There is no charge for routine processing.

If the change is more than one year after issuance, customers must submit an Application for Passport by Mail (DS-82), together with the passport to be changed, the documentation required, and new photos. The cost is the $75 application fee. Of course, if the requested data change is due to a printing error by Passport Services, there is no charge.

Those only needing to add visa pages to their passports must submit an Application for Additional Visa Pages (DS-4085). This service is free of charge.

Newborns: The Consular Section can issue a Consular Report of Birth Abroad of a Citizen of the United States of America (CRBA) and a passport to an American minor born in Croatia if at least one of the U.S. citizen parents meets the citizenship transmission requirements.

An appointment is needed to apply for a CRBA. Please call the Passport Unit at 385-1-661-2233 to set up an appointment.

To apply, both parents and the minor must appear in person. The following documentation should be provided:

  • Form FS-240 signed in front of the Consul. Available in the Consular Section
  • A completed passport application form DS-11
  • Birth certificate of the minor, translated into English if necessary
  • Passports of both parents
  • Marriage certificate (if applicable)
  • Evidence that the citizenship transmission requirements have been met
  • Application fee, payable by cash (dollars or kuna) or credit card
